A valid original passport that is not expired. We do not acceptphotocopies of passports unless they are certified by IRCC. If you do not have a passport, the following are acceptable forms of photo ID to bring:
Canadian Driver's license
BC ID card
BC Services Card with photo
Refugee Claimant Document
2. Payment
Exam fees and accepted types of payment can be found in the Clinic Fees tab, or by following the link below.
Current medications in their original packaging/bottles. Oral contraceptives (birth control) are not required to be brought to your appointment. Glasses/contacts (if applicable) as required for the eye examination.
4. Translator
English is spoken at this office. If you are not fluent in English you must arrange to bring someone with you for help with translation. We do not provide translation services at this office.
5. Related Medical Reports
If you have a medical condition, or have relevant testing information for your case, please ensure to bring copies of any relevant reports you may have. We will accept paper or digital copies of these reports.
